Getting Started

Illuminated Hearts provides free in home assessments.   We will meet with you and your family to jointly determine the scope of care by conducting an in-depth assessment of your (or your loved one’s) lifestyle and medical conditions. We then evaluate the kind of assistance that is needed.

During the meeting, we explain fully the services available, answer any additional questions you may have, and help with the important care decisions faced by families with a loved one in need.
