Here at Illuminated Hearts we hold Veterans in high honor. Illuminated Hearts works with veterans and their surviving spouses to provide quality in home care as an approved provider of the Veteran’s Choice Program. We also aide in procuring benefits to eligible veterans that will fund the cost of home care services, as well as refer veterans to professional resources to determine their eligibility to entitled benefits for home care needs.

Special Services

24 Hour Care

Illuminated Hearts can provide 24 hour care services if such a need arises. We will coordinate a team of experienced, compassionate and reliable caregivers to provide around the clock care for as many days needed.

Medical Alert Services

Have a piece of mind in knowing that you or your loved one will still be able to be monitored even when a caregiver is not scheduled. The Life Station Medical Alert service is available for medical emergencies.

Nurse on Call Support

Each person we provide care for has access to an on call nurse that can address any non-emergency health care concerns that arise. Our nurses also oversee and support the direct care worker staff. Should there be any unusual occurrences, our team of nurses can be reached for 24 hour on call support.

Family Room Portal

Illuminated Hearts offers the family of loved ones being cared for the ability to monitor how things are going through the online Family Room Portal . The Family Room Portal is an online platform that is updated with weekly care logs to give insight on the outcome of care. The Family Portal is interactive and family members are able to respond to care logs, make requests, and communicate through the portal. This service allows families to conveniently be included in the care coordinating process even if they are not able to be there physically.

Illuminated Hearts is an affiliate of Premier Urgent Care. PREMIER URGENT CARE is a walk-in facility which specializes in non-life-threatening illnesses and injuries. Premier can immediately accommodate any sudden health care concern. Premier’s healthcare facilities offer digital X-ray, EKG, lab service, IVs and prescriptions twelve hours per day, every day of the year. Premier’s medical staff is trained in Advanced Life Support, Pediatric Advanced Life Support and Basic Life Support. Premier treats patients quickly and effectively. In the majority of cases, Premier is a great alternative to a hospital emergency department. If you should need treatment from one your local specialists, Premier can schedule your appointment, deliver your medical records and have you on your way with no hassle.
